Some recent work has confirmed important information that is supported by comprehensive research reviews.9,20It was determined that three important factors help toexplain whether a person will drop out of, or stay with, an exercise program.5 These variables must be addressed if you seek to empower members for long-term exercise success.

You may be surprised to learnthat how wella workout enables rapid physical progress is not one of them. In fact, prescribing “by the book” to expedite cardiorespiratory and strength gains can often promote dropout.3 The factors (or resources) that can reliably predict long-term exercise success or failure are as follows:

1. The amount of social support received by the new participant (from a spouse, partner, family, friends, peer group or other exercisers)

2. The participant’s ability to use self-management and self-control strategies (e.g., positive self-talk, goal-setting practices, relapse prevention or dissociating from discomfort)

3. The participant’s ability to tolerate physical discomfort

Of the three, ability to tolerate discomfort may be the most important (and least attended to by exercise professionals).Obviously, all the knowledge about promoting physiological gains through theoretically valid exercise programming will do nothing if the intensities, durations and frequencies prescribed cannot be tolerated well enough for a training effect to take place and be maintained.

When you exercise, your heart beats faster to meet the demands of your muscles for more blood and oxygen. The more intense the exercise bout, the faster your heart beats. Thus, all factors considered, monitoring your heart rate during exercise provides a very accurate reflection of the metabolic intensity of the exercise. Several techniques can be used to determine an appropriate target heart rate.

Exercise intensity can be prescribed in terms of heart rate (HR) by using specific heart-rate values that are approximately equal to 60 to 80 percent of V02max. One method involves monitoring HR at each stage of a maximal graded exercise test (GXT). Heart rate is plotted on a graph against the V02max (or MET) equivalents of each stage of the test in order to define the slope of heart rate response to exercise (refer to Figure 1). From this relationship, the exercise heart rate associated with a given percent of V02max can be obtained. While this method is generally the most accurate approach for determining an appropriate level of exercise intensity, it is often impractical since it requires a person to complete a maximal GXT — with its associated logistical and safety considerations.

A line of best fit has been drawn through the data points on this plot of heart rate and oxygen consumption data observed during a hypothetical maximal exercise test in which V02max was observed to be 38 ml per kg -1 per min -1 and maximal heart rate was 184 beats per/minute. A target heart rate range was determined by finding the heart rates that correspond to 50 percent and 85 percent of V02max.

For this individual, 50 percent of V02max was approximately 19 ml per kg -1 per min -1 and 85 percent of V02max was approximately 32 ml per kg -1 per min -1. The corresponding heart rates are approximately 130 and 168 beats/minute. The target heart rate range for this individual is 130 to 168 beats/minute.

Adapted from ACSM’s Guidelines for Exercise Testing and Prescription, 5th ed. Philadelphia, PA: Lea & Febiger, 1995.

Another method for assessing exercise intensity is based on the observation that 70 and 85 percent of maximal heart rate is equal to approximately 60 and 80 percent of functional capacity (V02max). For example:

THR range = maximal HR x 0.70 and 0.85

where:

THR = training heart rate

maximal HR = 220 – age (low estimate)

or

210 – (0.5 x age) (high estimate)

The third method for determining the exercise heart rate for training is to calculate the heart rate reserve (HRR). Developed by Doctor, the HRR method of determining the training or target heart rate range requires a few simple calculations:

First, subtract your resting HR from your maximal HR to obtain your HR reserve.

Then, calculate 60 percent and 80 percent of your HR reserve.

Then, add each value to your resting HR to obtain your THR range.

Folic acid, a B vitamin, is known to be protective against certain birth defects — neural tube defects (NTDs) — that develop early in pregnancy. A new study published in the August 1999 issue of The American Journal of Clinical Nutrition confirms that folic acid fortification significantly raises the folate levels of women and suggests that only an extra 100 micrograms (mcg) of folic acid a day could benefit women in their childbearing years. Folic-acid-fortified products include breakfast cereals, all enriched breads, pasta, all enriched flours, and other food products.

Among the common NTDs are anencephaly and spina bifida. In spina bifida, the spinal column fails to close and is exposed, while anencephaly means that most or all of the baby’s brain is missing. The Food and Drug Administration (FDA) recommends that women who are pregnant or capable of getting pregnant consume 400 to 600 mcgs of folic acid before and during pregnancy.

Researchers from the University of Ulster, Northern Ireland, and Trinity College, Ireland, investigated the impact of products fortified with little amounts of folic acid on folate levels and whether their elimination from the diet would have a negative effect on those who normally consumed them.

Fifty-one women between the ages of 17 and 40 who were neither pregnant nor scheduling a pregnancy were instructed to eliminate folic acid-fortified products from their diet for 12 weeks. The researchers designated the 21 women who usually ate folic-acid-fortified foods at least once a week “consumers.” The 30 “nonconsumers” ate such foods less than once a week. All the subjects had an initial folate status within the normal range, which was assessed by measuring red-blood-cell (RBC) folate concentrations.

The exclusion of fortified foods (primarily breakfast cereals) resulted in a significant decrease in the average total folate intakes and RBC folate concentrations in consumers, but not in the nonconsumers. Past research has shown that the higher the RBC folate concentration, the lower the risk for NTDs. The findings seem to validate the fortification of breakfast cereals, since such products were the only folic-acid-fortified foods the subjects consumed before participating in the study. When fortified cereals were removed from “consumer’s” diets, their total folate intakes and RBC concentrations dropped significantly.

The report also showed that those who regularly consume foods fortified even at low levels will have considerably higher intakes than those who eat them less frequently. The researchers found that the women who ate folic acid fortified foods at least once a week had on average a 35 percent higher intake of the vitamin.

Folic acid fortification became mandatory in 1998. The FDA requires that 140 mcgs of folic acid be added to every 100 grams (about 3 1/2 ounces) of enriched grain product.

But folic acid fortification is not without controversy. While folate fortification may help prevent neural tube defects (NTDs), there is also the possibility that symptoms of vitamin B12 deficiency may be masked, which may prevent its early detection. Many older people and others who are genetically predisposed may not absorb adequate amounts of B12 from their food. Raising the levels of folate fortification too high could create a serious problem in that population.

It is because of the potential detriment to the elderly that Dr. Victor Herbert of the Bronx Veterans Administration Medical Center opposes the current folic acid fortification policy. Although the actions taken by the FDA are considered by many as promising in the fight against NTDs, Dr. Herbert states: “Folate fortification alone helps a few but harms many. Whenever there is fortification with folic acid, foods should also be fortified with vitamin B-12.”

We spent hours together in the basement of an apartment complex in the north Bronx, all screeches and giggles, pretending to run from a monster rumored to be hiding there.

Other days, we idled away hours at the swing in the woods by the Hudson River, sailing over the treetops together on the well-worn rope. When we grew older, and more rebellious, we huddled together in the schoolyard, sharing gossip, secrets and cigarettes.

To promote awareness of prostate cancer and the dangers of high cholesterol, the Employee Wellness Warehouse at AlliedSignal offered onsite prostate cancer screenings and cholesterol testing sessions, and held informational sessions on the topics. Many people “don’t have time to go to the lab, so we brought the lab to them,” says Matthew Moreno, exercise specialist for the Employee Wellness Warehouse. Employees enjoyed the convenience of onsite screening, which also educated AlliedSignal employees, spouses and Wellness Warehouse members on how to prevent prostate cancer and lower cholesterol levels. An onsite electronic monitor system, flyers, banners and mailings were used to promote the week-long event.

After the cholesterol information and testing sessions, and the prostate-specific antigen (PSA) blood testing, individuals received a detailed explanation of their PSA and/or cholesterol values. Recommendations for further examination were made for individuals who fell into high- and mid-risk categories.

The primary purpose of Wellness Warehouse, according to Moreno, is to “promote wellness for the employees at AlliedSignal,” so the screenings would have been successful even if they only prevented the occurrence of prostate cancer in one individual.

Another goal of the screenings is to reduce insurance rates, since statistics show that healthcare costs can be reduced up to $3,005 per individual each year if prostate cancer is prevented. The company could save more than $60,000 in medical costs if one coronary bypass surgery is prevented.

Although financial benefits resulted from the screenings, more important is the knowledge acquired by employees about prevention. Prostate cancer is almost entirely preventable. The event resulted in 224 prostate and 136 cholesterol screenings — a total of 360 employees educated about the long-lasting benefits of good health.

You’re at the playground or pediatrician with your baby, and you see another mother eyeing your little one. “How old is he?” is her first question. “What was his birth weight?” is often the second. “What does he weigh now?” comes soon after. New Moms, especially first-time Moms, are often terribly concerned with their baby’s weight and eager to compare with other Moms. If your baby is growing normally and your pediatrician is not concerned about his weight, why should you be?

It starts with birth weight. There is an unwritten scale of birth weights that seems to serve as a status check for new Moms. Seven or eight pounds is a nice size. Nine pounds is impressive. Bigger than that, and you’re bound to entertain some delicate questions about your method of delivery and amount of painkillers. My own kids were both in the eight-pound range. After my sister-in-law delivered a nine pounder, she informed me that it was my turn to have a heavier baby, as if we were in a contest to deliver the chubbiest baby. No thank you!

Since your baby can’t tell you when she’s had enough to eat, Moms often try to use baby’s weight gain, or lack thereof, to evaluate if they are feeding properly. That is understandable, but not dependable. Babies may be born large because of their mother’s diet during pregnancy, but grow at a slower rate during the first year as they head towards their genetically programmed natural size. Along the same lines, a baby born small due to prematurity or other factors may be in the 95th percentile at the end of a year. According to my pediatrician, percentile weights will fluctuate for the first two years as your baby “finds her place.” Don’t assume that your baby’s slow or fast growth means you are not feeding her properly. If you are worried, talk to your pediatrician and, if applicable, La Leche League or a lactation consultant.

Then there is the gender issue. My kids were both in the 90-to-95th percentile for weight in their first year. My son was greeted with cheers of “Call Joe Paterno! Football scholarship! What a bruiser!” About my daughter, on the other hand, I was warned “You’re going to have to watch that one. Looks like she likes to eat.” Mind you, they both followed a nearly identical growth curve, with my daughter consistently about one pound less than her brother was at the same age. Why is a big boy good and a big girl bad? Even now, people are noting with obvious relief that my daughter, now two, has “slimmed down.”

Don’t worry so much about your baby’s weight. Your pediatrician will alert you at your well-baby visits if your baby is gaining too quickly or too slowly. A big baby will not necessarily be a fat child; a small baby will not necessarily be a short adult. Babies come in all shapes and sizes — just love them as they are!

Pharmacies make is more reliable, easier, more convenient for you to order tramadol online with no a prescription, allowing you to economize your hard-earned capital for another significant necessities you may perhaps need.

In a Harvard Medical School sponsored study that was published in the October 1, 1998 edition of the prestigious medical journal, The Annals of Internal Medicine, concluded that the long term use of multiple vitamins reduced colon cancer risk by 75%! The risk was reduced by 20% after 5 years and 75% after 15 years of taking multiple vitamins.1

A recent International Journal of Cancer article revealed findings of a 50% reduction in risk of Nasopharyngeal Cancer in those individuals who took supplemental vitamins.2

Vitamin E
People with higher vitamin E intake are less likely to develop cancer.3 An eight year long study of over 36,000 patients was published in the Journal of the National Cancer Institute. It showed that those with the lowest levels of Vitamin E had a greater risk for developing many types of cancers.4

Scientists have related low Vitamin E levels and low intake of Vitamin E to colon cancer, breast cancer, lung cancer, oropharyngeal cancer, cervical dysplasia, and cervical cancer.5

The National Cancer Institute and the National Public Health Institute of Finland conducted a large prevention trial that showed a 41% reduction in prostate cancer death rate in those taking Vitamin E supplements at five times the RDI dosage.6

Selenium
The U.S. Food and Nutrition Board established the Committee On Diet and Health which analyzed diet and major chronic diseases. This group of experts stated in their report “Low selenium intakes or decreased selenium concentrations in the blood are associated with increased risk of cancer in humans.”7

Selenium offers significant protection against lung, breast, colon and liver cancers.7,8,9,10,11

The risk of advanced prostate cancer for men who took 200 mcg/day of selenium was reduced by 65% in a study published in the Journal of the National Cancer Institute.12

In a study sponsored by the National Institute of Health and the American Cancer Society, Dr. L.C. Clark published an article in the Journal of the American Medical Association studying the effect of 200 mcg/day of selenium. He found a 49% reduction of the cancer death risk among those taking the selenium supplement, with 63% fewer prostate cancers, 58% fewer colon cancers, and 45% fewer lung cancers.13

Vitamin C
Vitamin C supplementation has been shown to protect DNA from damage according to the National Academy of Science.14

Vitamin C blocks formation in the stomach15,16 of nitrosamines, well known cancer causing compounds.

Vitamin C has provided significant protection in lowering cancer risk in 33 studies,16with research showing a consistent effect on oral, pharyngeal, esophageal, and stomach cancers.

The retrial in Libya of five Bulgarian nurses and a Palestinian doctor who have been accused of deliberately contaminating hundreds of Libyan children with AIDS, reopened this week.

Judge Mahmud al-Huweissa has called for the process to be sped up and has adjourned the trial to June 20 after a brief session and ruled that any petitions will in future need to be filed in writing.

The judge says weekly hearings will now take place because the case has “dragged on too long” but has rejected an application for the accused to be released on bail on the grounds that the defendants might be harmed by the victims’ relatives.

Bulgaria has welcomed the decision and says they hope that the testimony of all the witnesses called by the defence and the arguments it will present will be reviewed with the maximum objectivity and the case decided with the shortest possible delay.

The team of Bulgarian defence lawyers for the five nurses called for a new expert opinion on what caused the hospital AIDS outbreak that infected the children, as the two existing assessments are incompatible.

Libyan experts said the outbreak was knowingly sparked by the nurses who have been in jail since 1999.

The nurses and the doctor were condemned to death in May 2004 at an initial trial in the eastern city of Benghazi after being convicted of infecting 426 Libyan children with AIDS while working at the local hospital.

An international outcry followed as the trial was widely perceived as unfair and human rights groups accused the Libyan authorities of prosecuting the foreigners in a cover-up.

At their first trial, scientist and co-discoverer of the AIDS virus, Luc Montagnier, and Italian Professor Vitorio Colizzi said the disease was rife before the nurses’ arrival in Libya and was due to poor hygiene in the Benghazi hospital.

Defence lawyers also say that the incriminating testimony of Libyan medical experts was wrong and should be re-evaluated and they have urged the court to review the “pressure” brought to bear on the accused after their arrest in 1999.

They have asked the court to accept written testimony from a Bulgarian engineer Smilian Tachev, who was arrested together with the nurses but freed six months later who has said the nurses were tortured in detention.

Amnesty International has also said the women reported being tortured with electric shocks and beaten until they confessed.

Palestinian doctor Ashraf Hajjuj has complained about the conditions in which he is being held, and claims he is being discriminated against.

One of the defence lawyers, Thahiba Mohammed Moussa has stood down from defending the doctor and has reportedly joined the team of lawyers representing the victims’ relatives.

The nurses are refusing to talk to Bulgarian journalists in the Judeida prison on the grounds that after eight years they have nothing to say.

The six proclaim their innocence and appealed to the Libyan Supreme Court, which ordered a new trial last December 25.

In December 2005 an international fund to fight AIDS in Libya was set up by Bulgaria in partnership with the European Union and the United States but meanwhile another child has died bringing the death toll to 52 with 50 more children giving cause for concern.

Bulgaria said the fund will ensure permanent medical care for AIDS patients, raise the standard of the Benghazi hospital to international levels and provide financial help to families with sick or dead children.

Bulgaria has rejected the idea of paying compensation to the families, or writing off some of Libya’s debt, saying such a move would be seen as an admission of the nurses’ guilt.

It is reported that the Libyan families have demanded $10-million in compensation for each of the 426 children contaminated with AIDS.

The families’ demand was formally rejected by Bulgaria, which believes in the nurses’ innocence.

The trial has become the equivalent of a diplomatic “High Noon”, with the U.S. and European Union suggesting that future relations with Libyan leader Gadhafi could depend on the verdict.

In general, the medical community is in agreement that a multi-pronged approach to ADHD combining medication with behavioural therapies works best. Although family environment does not cause ADHD, it certainly can influence behaviour. Behaviour therapy usually involves family training through sessions with a qualified therapist.

Although medications are only one-prong of the solution, they often generate the most concern from parents. The goal is to find a medication and dosing schedule that gives maximum results with minimal side effects. It is clear that many children with ADHD improve their performance and behaviour when treated with medications, but any medication for a chronic condition should be given with particular care.

Tried and True

Ritalin and other central nervous system stimulants, such as Adderall and Concerta, are the most commonly prescribed ADHD medications. Although media coverage of ADHD is a product of recent years, Ritalin has been around far longer. It has been dispensed from pharmacies for over four decades.
The benefit to an older drug is that the cost is lower and we know the side effects. Previously, one of the greatest downfalls to Ritalin was the need to take additional doses every four hours, but new long-acting formulations for central nervous system stimulants are now available for children who need extended coverage.

On the upside, multiple doses are not needed and parents control dispensing at home, giving children the privacy of taking medication without a trip to the school nurse. On the downside, because formulations work using a drug-release system, you cannot cut the dosage and you have to make sure your child does not chew the pill.

In With the New

Many US pharmacies have begun stocking Strattera, the first non-stimulant medication approved by the Food and Drug Administration for ADHD. Choosing a drug new to the market has its advantages and disadvantages. Advantages include shown success in controlling symptoms without causing insomnia and a less complicated refill process, since Strattera is not a controlled medication as stimulants are. The disadvantages: the price tag is high and as with any new medications that is widely dispersed for the first time, certain side effects might not yet be known. If your child is taking Strattera make sure to report any reactions you think may be associated with the drug.

A recent article in the excellent Journal of Complementary Medicine described the benefits of selenium in the form of selenomethionine in women with benign breast disease.

Benign breast disease is also known in medical terms as:
Chronic mastitis
Cystic fibrosis
Hormonal mastopathy
Benign mammary dysplasia
Mammary intraduct epithelial hyperplasia

The underlying inflammatory process in the breasts that produces it can produce breast cysts, destruction of healthy breast tissue and breast cancer. It is considered a precursor of breast cancer and thus should be carefully monitored as the transition to cancer can take 15 to 20 years. Thus if it is diagnosed in a woman in her thirties, it may not lead to breast cancer until she is in her sixties. In women aged 45 or less, who have breast cysts, the risk of breast cancer is increased by a factor of 6, compared to women without breast cysts.

The symptoms of benign breast disease include:
Breast pain and sensitivity
Swelling of the breasts
Breast cysts
Induration or a solid hard texture to the breasts especially towards the armpits

A clinical trial of 135 women with severe benign breast disease used 100mcg daily of selenomethionine and produced very good results in controlling the breast inflammation.

It is thought that selenium is able to exert significant protection against free radical and oxidative stress in the breast tissue. It is important to note that the measurement of selenium levels in the serum of a blood test is not an accurate guide to the dose of selenium required to act as a preventative agent, which is in excess of current nutritional guidelines. This is particularly significant in the context of the low selenium content of Australian and New Zealand soils.

Mr. James Fitzherbert who is a consultant surgeon, suggests that selenium supplementation may be much more effective if it is started at the relatively young age of 18 to 25 years, because at this time the breast tissue has maximum susceptibility to free radical damage.

He suggests that young women with benign breast disease may need more attention than older women to reduce their risk of breast cancer. He also suggests that women taking combined Hormone Replacement Therapy (HRT) may need to take selenomethionine as a preventative agent to the degree where it becomes essential.

Selenium is a vitally important mineral for the human cell and can be beneficial for the following –
Recurrent viral infections

Thyroid disease including goitre & autoimmune varieties such as Hashimotos thyroiditis
Inflammatory diseases
Perhaps its greatest role for the future will be to reduce the risk of cancer.
